We would like to share with you a new resource for your Continuum of Care work. We have created an organizational tool and checklist to assist you in writing your Continuum of Care report that includes your asset map, gap analysis, plan and cost saving information. We know there is a lot of information to include so this guide can serve as a checklist for you to ensure you cover all the information.
This tool is a resource not a requirement by OJJDP. Please be sure to include all the information in the report as required by the solicitation. We hope this tool can help organize the reporting of all your hard work!
