Program Evaluation: Assessing Your Program - Sample Sustainability Action Plans

Resource Type

Developing strong action plans is important for meeting goals and objectives. The Center for Public Health Systems Science at Washington University at St. Louis has developed examples of action plans for the Program Evaluation, Partnerships, and Communications domains. The plan includes steps to achieve the objectives. Examples include the need to be very specific and make sure to include important sub-steps. A good rule of thumb is that anyone should be able to pick up this document and understand what needs to happen in order to reach the desired objective. It is also important to determine who will do the work and ultimately ensure the work gets finished.  Defining success is vital and entails understanding what metrics will be used to track progress related to the completion of each step and understanding when it is time to move on to the next step. Identifying sources of non- financial resources is important. The plan should include a timeline with interim due dates for each activity.
